5. The fact that a person may have some organs that is constitutionally weaker than others.
The atrial septal defect is a malformation of the heart present at birth which is often referred to as a 'hole in the heart'. There is a defect in the wall between two of the chambers of the heart (atria) which do not normally interconnect. As a result some of the blood which should be pumped to the lungs to collect oxygen may be short-circuited and passed round the circulation of the body again, without going to the lungs first. As a consequence the heart may have to work harder than usual to pump sufficient oxygenated blood round the body. If the hole is small it may cause very few problems and gradually close as the child grows. If it is larger the heart may be under strain and more active medical or surgical treatment will be required. The two other chambers of the heart (ventricles) may also be affected. This is known as ventricular septal defects. It is usually diagnosed when an abnormal sound is heard by a doctor listening to the heart with a stethoscope. The 'murmur' is made by the turbulence of the blood as it passes through the hole. Children with mental handicap caused by factors operating before birth may have an associated abnormality of the heart. It is, for example, more common than usual in people with Down's syndrome, Smith-Lemli-Opitz syndrome, de Lange syndrome, Goltz's syndrome and many chromosome abnormalities. The ventricular septal defect is a type of congenital heart disease in which there is a hole in the heart due to a defect in the wall between the two ventricles. This reduces the efficiency of the heart, which has to work harder, and is under strain. This may lead to heart failure and other complications. Small defects may close spontaneously but most require operative interventions which may involve high risk. This type of congenital heart defect is quite common in Down's syndrome and in many other conditions associated with mental handicap. (Hunt & Meacham, 2001)
