After adoption, the infectious disease consultant is usually asked about the adequacy and efficacy of the child’s vaccination procedures and records and assists in further screening, evaluation, management and control of the infectious diseases found in the child to provide him or her with a better future.
Severe infections such as outbreaks of tuberculosis, varicella zoster virus and scabies have been described in a residential facility for children tested HIV positive. Geoghagen et al. (2004) reviewed results and management for cases of acute TB, varicella zoster virus and scabies in 24 HIV positive and abandoned children. Control of outbreak of such infections and the related challenges have been described and both clinical and WHO criteria were used for the diagnoses of the diseases. 22 children survived the infections and 16 children were found to be fully immunized against the infections. The children who died of tuberculosis were not on antiretroviral triple therapy and nearly 68% of the children developed chicken pox and the varicella outbreak affected the caregivers as well. Many residents also developed a dual chicken pox and scabies infection and the paper naturally expose the lack of knowledge in disease outbreaks and control. The emphasis of the authors is on improvement in immunization coverage for children and staff and educating staff about outbreaks and possible controls of disease outbreaks. Geoghagen et. al (2004) write, ‘Appropriate screening for infection/disease for all susceptible persons is essential along with timely reporting of outbreaks/reportable diseases’. They also highlight the need for increased awareness of the possibilities of rapid spread of acute infections in children with HIV/AIDS, who are particularly vulnerable and living in close proximity.
In an important paper, Francis et al. (2002) point out that there has been a recent resurgence of tuberculosis and it is a public health concern, especially in cases of children. The paper discusses problems of tuberculosis in children of Haiti and policies for control as laid down by Centers for Disease Control and Prevention (CDC) and World Health Organization (WHO). The methods of control and eradication of infectious diseases include recommendations for screening for such diseases in prisons, shelters, and other congregate-living facilities, and screening in orphanages and residential centers can also be carried out. This paper is mainly based on data obtained through screening for tuberculosis and report cases of 445 orphans who were screened. Symptoms of tuberculosis were identified as fever, night sweats, and cough for more than a month and 44% cases were found to possess some form of the disease.
